The Care we Provide

At Active Care Partnerships we provide care and support for a number of different conditions in a variety of settings. Your social worker or care manager will be able to help you in deciding which type of care and support is most appropriate to your needs.

Residential Homes

Our residential homes are designed for those people who find it difficult to care for themselves at home but are still looking to maintain some independence. Our approach is to create a “home-from-home” environment whilst providing high quality personal care.

Our centres have shared communal areas whilst each individual enjoys their own bedroom and en-suite bathroom. Personal effects and furnishings are always welcome. Service users are also encouraged to participate in the decoration and personalisation of their own bedroom.

Specialist aids and adaptations are provided for needs arising from any physical disabilities.

Our holistic approach to care emphasises the importance of social interaction and activities. Our people are very experienced at listening, understanding and responding to the needs of service users. As part of our person centred approach to care service users are encouraged to work towards their aspirations following an agreed timetable with the support they require.

As with all of our centres, residential homes are staffed 24 hours a day by trained carers. Their focus is on ensuring the personal needs of each and every service user are taken into account.

Nursing Care Homes

In addition to the care we provide at residential homes, our nursing care homes provide 24 hour specialist nursing care. This care may include respite care and palliative care tailored to the needs of the individual.

The team are trained to work closely with various clinical disciplines and take a holistic approach to the care they provide. We believe it is important that our service users keep as physically and mentally active as possible to maintain their independence and quality of life.
